You have been referred because of concerns about cancers which have occurred in your family. To help us give you the most appropriate advice, it is helpful to collect some information about your family. Please provide as much information as you can on your family members, including those who have not had cancer. For those family members who have had cancer, please include the type of cancer and their age when diagnosed. This information helps us to assess the chance of there being an inherited predisposition to cancer within your family.
You can provide this information securely through this online tool. Once you have completed the form, you can submit it securely online or you can print and post it to The Cancer Genetics Service. You will need the answer to your secret question provided during the registration process to submit the form.
